I keep my life compartmentalized for the most part so nobody even knows I frequent a message board at all, much less that I've slapped up half naked pictures for the enjoyment of internet strangers.

When Sack stopped posting I searched for Nebraska news/obituaries for people named Zack wondering if he has offed himself or joined DixieBandit. He seemed to be going through a very rough patch in his life. I was unsuccessful in my search. I am happy for that. I really don't want people dying. He probably realized no one thought he was funny, because when he was going through his life problems people were very mean in their insults to him. I'm not saying he didn't deserve it. He did wish death on Brian while he was in Afghanistan, and pretty much was a white supremacist. The thing about Sack was that he was a deeply troubled and self-hating individual. This routinely came across as racism and antisemitism. Basically being a bully because it was the only mechanism he knew. Then he returned one day having read an Eckhart Tolle book and thought he was the fucking Dalai Lama. No one bought his shit and he returned to his old style almost immediately. Then peaced out.

I have a genuine question for our American Idiots. I was over on Ned Hardy (don't judge me) looking at a thread that showed hospital bills for some pretty basic stuff that totaled in the tens (and tens, and tens) of thousands of dollars. What happens if you don't have insurance? What happens if your insurance doesn't cover something? Do you actually have to pay shit like that? How are you not all carrying hundreds of thousands of dollars of medical debt for your whole lives? I'm sorry if that sounds naive or sheltered, I just don't understand how your system works. Why on earth would you guys have babies when giving birth costs 50 grand?
Typically if someone is self-pay, they discount the overall cost quite a bit, but the debt is still crippling. Typically the markup on medical care is so high because most insurance companies will pay back 50% or less of the billed out amount. Medicare and medicaid pay out about 50% less than private insurance. So the costs on the bill are jacked up to compensate for the extreme lack of reimbursement. Hospitals rely huge on private insurance to get most all of their money, because they know the crack heads looking for a fix and the people who don't have any money or insurance who use the ER as a walk-in clinic won't pay a dime. They try to recoup the costs somehow and that is you and me. By law, an emergency room cannot refuse a patient regardless of ability to pay and the crackheadsn know that. So hospitals eat a lot of costs. They can bill and bill all day long, but if you don't have money, you don't have money.
It is pretty fucked. I've viewed a lot of hospital financials in my job and to answer part of your question, charity care expenses amount to about 5% of the revenues in general (well, according to a quick cursory look at some systems in my home state- I'm sure that number will range). I work in the patient payment part of the healthcare revenue cycle. Overall it shocks me how many hospitals, smaller rural and critical access providers in particular, are bleeding money today.
