More than a decade ago when I was doing my student nurse stuff at the hospital, I learned it was normal practice for nurses to refer to people as baby mama or baby daddy. It references the relationship to the patient but does not give details as to the relationship between the parents. It can be incredibly complicated and at the end of the day, what hospital staff cares?
At the hospital my kids were born at, there was someone on site that officially registered birth certificates. In that sense, it probably does matter.
